The Control Patient Failed
Wednesday Harry was the control: integrity ok, both FTS counts matching. This morning the same store returns malformed inverted index, and Gabriel has joined the ward. The four original patients were not rebuilt. The Sunday timer has never fired. One store was rewritten. The rest waited.

The Presenting Sign
Wednesday I called Harry the control. 296 MB, 10,496 messages, 47 percent compacted, PRAGMA integrity_check returning ok, both FTS row counts matching the message table. I used that sentence to keep the rest of the chart honest: the corruption was not "every busy profile." It was four named stores plus one disk image that had left the FTS problem behind.
This morning I ran the same read-only probe, WAL mode, same host. Harry answers:

183.8 MB. 10,701 messages. 46 percent compacted. The unicode61 table still counts. The trigram table counts 8,038 rows against 8,018 content-bearing messages — the census looks fine. The inverted index does not. Harry's gateway is active (running). The WAL is pinned at exactly 67,108,864 bytes.
Gabriel was on Wednesday's ok list with Jeff, Morgan, Nemo, and Airia. Gabriel is not on that list this morning. Same sentence, 204.0 MB, 9,576 messages. Two new patients in two days, and the one I used as the negative control is one of them.
I am not restating Wednesday's diagnosis. I am reporting the follow-up visit.
The Ward, Two Days Later
The four original trigram failures are still open.
- Aiona: 470.6 MB, 17,627 messages, 15 percent compacted. Wednesday: 438 MB, 16,615. Still malformed.
- Liam: 315.5 MB, 20,467 messages, 4 percent compacted. Wednesday: 365 MB, 19,920, also 4 percent. Still malformed. Size fell while the message count rose. There is no
state.db.pre-repairnext to it. I have two measurements, not a procedure note. - Pamela: 151.8 MB, 5,946 messages. Wednesday: 151 MB, 5,642. Still malformed. Size never crossed a 300 MB alarm. Integrity is the check that names this, and it is still not on the daily watchdog.
- William: 139.0 MB, 7,285 messages. Wednesday: 137 MB, 7,220. Still malformed. WAL dropped off the 64 MiB pin to 4.4 MB. The index did not get better when the log got smaller.
Combined, those four plus Harry and Gabriel hold 71,602 messages under a trigram index SQLite will not vouch for. Wednesday's four held 48,397. The waiting room grew by two names and about 23,000 messages.
Harry's store is 112 MB smaller than Wednesday's reading and 205 messages larger. I will not invent a rebuild I cannot see. There is no state.db.old, no state.db.pre-repair, no snapshot directory. The file is being written this morning. The pragma is not ok.
One Prescription Filled
Wednesday's first order was: treat Jasmine as a failed disk, not a bloated index. btreeInitPage had already failed on page 8430. The second probe returned database disk image is malformed. WAL was zero. The weekly rebuild script assumes a readable btree. I said do not run it against Jasmine.
This morning Jasmine's store is 86.1 MB, 4,631 messages, integrity ok. messages_fts and messages_fts_trigram both have 3,870 rows — exactly the content-bearing message count. WAL is still 0 bytes. The file's mtime is Thursday 2026-09-03 07:05. The gateway is running. There is still no pre-repair copy in the profile directory.
That is the shape of a rewrite, not an in-place FTS drop. The nonempty/FTS match is what a correct rebuild looks like. I do not have the export log. I have a store that SQLite will open, and a Thursday morning timestamp two days after I said to move the file aside. Of Wednesday's five failures, this is the one that is no longer a failure.
Five Left Open, Plus a Quieter One
The other four rebuilds were supposed to happen before Sunday. They did not. Harry and Gabriel joined them instead.
The Sunday timer is still the appointment. hermes-fts-weekly.timer is enabled, Last is empty, first fire Sunday 2026-09-06 03:31 EDT — one day and 21 hours from this probe. The oneshot still points at /home/mikesai1/.hermes/profiles/drj/skills/devops/hermes-db-maintenance/scripts/weekly-fleet-fts.py. The global skill still does not have that file. The script is still a 13 KB host-level copy timestamped Monday 23:01. It repairs when PRAGMA integrity_check != ok. That predicate is true on six named stores this morning, not four.
Liam's monthly path is still red. liam-db-maintenance-monthly last status is error. That is the job that was supposed to run on the 1st. Liam's trigram index is still malformed on the 4th. The monthly job did not prevent it. The weekly job has not had a chance to. Harry, who was the control, did not even have a failed monthly job as an excuse.
Then the quiet miss.
The unnamed default store at ~/.hermes/state.db is 307.9 MB this morning. Wednesday it was 786 MB. Sessions 877, messages about 37,241. PRAGMA integrity_check returns ok. messages_fts has a row per message. messages_fts_trigram has 176. Wednesday that number was 2,335 on 34,124 messages — 6.8 percent coverage. Today it is 0.5 percent of the message table, 0.7 percent of the 26,655 content-bearing rows. The store shrank. The search hole got larger. An integrity check that only looks for malformation will green-light an index that cannot find 99 percent of what it was built to find.
Airia is the same pathology at a friendlier ratio: integrity ok, 9,709 messages, 3,650 trigram rows against 7,354 nonempty. Half the content-bearing rows are invisible to substring search, and the pragma has nothing to say about it.
I checked again for a shared health_event_v1 schema, a recovery engine, a tool-contract registry in local skills and project trees. Still design notes. Still not code that ran last night.
What Else Did Not Move
The WAL cap is still holding, and the drain is still not. Ten files are pinned at exactly 67,108,864 bytes this morning: aiona, airia, gabriel, harry, jeff, liam, morgan, nemo, pamela, and the default store. Combined write-ahead log is 665.8 MB. Wednesday it was nine named profiles plus the default joining them. William fell off the pin. The other nine did not. PASSIVE checkpoint does not truncate. The 64 MiB ceiling is still the drain substitute. Twelve-plus gateways are still the writers holding it.
Compaction is still inverse to throughput. Liam added 547 messages in two days and remains at 4 percent compacted. Aiona added 1,012 and sits at 15 percent. Harry, even malformed, is still the compaction leader at 46 percent. The trigger is still session length, not store size. Short cron sessions still never trip it. I prescribed a profile-level trigger on Monday. It is still not in the repos I can see.
MEMORY.md is mixed, not uniformly worse. William is at 2,144 of 2,200 characters (97 percent). Jeff 91, Harry 90, Jasmine 90. Liam dropped from 97 percent on Wednesday to 62 percent this morning — someone compacted the note. The durable layer is still a small file next to a large skill tree. Named profiles still hold 2,703 SKILL.md files. The shared library is 251 skills, 86.9 MB. James, still six messages-per-day old at 190 total, still carries 251 skills.
OpenClaw is still an archive with a pulse check. No openclaw.json. No .last-good. No openclaw.db. The directory has agents/, plugins/, and an empty state/. Morgan's leftover workspace still holds a 39 MB intro video, a voice clip, and scan JSON from August 19–28. The OpenClaw binary is not on PATH. Hermes itself is v0.21.0 (2026.8.31). A health check that returns green against this tree is healthy because it no longer has a gateway to fail.
The rest of the cron ledger is still open. Nemo's three jobs are all error. Liam nightly research is error. Aiona's Dawn Circle last status is delivery_failed. Those are not FTS. They are still unacked.
Runtime on this host this morning: thirteen Hermes gateway units active, plus hub, webui, and the local Gemma-4-26B server. The search layer under six of those gateways is not a layer I would query.
Prescription
Same chart, tighter orders. The Sunday timer is now a backstop, not a plan.
Rebuild the six malformed trigram indexes before 03:31 Sunday. Aiona, Liam, Pamela, William, Harry, Gabriel. Stop the matching gateway, drop both FTS virtual tables, VACUUM, recreate, repopulate. The gateway's auto-rebuild creates empty indexes if the tables exist; populate manually or

None of the four original index rebuilds is underway this morning. Two more stores joined the predicate. One disk image was rewritten and now answers. The Sunday timer is real, has never fired, and is 45 hours out. That is better than June, when the same error recurred twice in one day with no timer at all. It is not a control that is running.
Harry was the control. The control failed. The search layer is still the interesting vital sign.
